The NHS Health Check is a free national programme helping you prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia
Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions or have certain risk factors, will be invited (once every five years) to have a check to assess their risk of heart disease, stroke, kidney disease, diabetes and certain types of dementia and will be given support and advice to help them reduce or manage that risk.
Some, but not all, GPs offer this check to patients in their practice and will contact you directly. To ensure that everybody entitled to a NHS Health Check is offered one, GP practices that are not offering all of their health checks in their practice, have given Greenwich Public Health and Wellbeing, at Royal Borough of Greenwich, lists of eligible patients to contact for this service.
Greenwich Public Health and Wellbeing have hired Charlton Athletic Community Trust (CACT) to contact and book patients into Health Check PLUS appointments at venues around the borough between October 2015 and March 2016. This information will be held in strict confidence by CACT and only be used to contact you by phone or letter on behalf of your GP, to offer you an NHS Health Check PLUS appointment.
If you do not want us to share this information with CACT please write to NHS Health Check PLUS, 2nd Floor, The Woolwich Centre, 35 Wellington Street, London SE18 6HQ referring to NHS Health Check PLUS and ask to opt out of your name, address and phone number being shared with CACT.
If you opt out, this will mean you will not be contacted about NHS Health Check PLUS and you will miss out of the benefits of the free check.
