Overview

The Dynamic Support Register is for people who have been diagnosed with a learning disability, autism, or both.

It is a list of people who need support as they're at risk of going into hospital if they do not get the right care and treatment in the community.

The register holds information about the young person and if they need more community support in any areas of their care.

The register helps:

  • professionals understand your child's needs and help identify signs that could lead to a crisis
  • your local NHS understand the needs of people with a learning disability and autism and can ensure their care network can find the right support for the child at the right time
  • services work more closely together to manage the support people get and ensure good outcomes in the local and wider community
  • local support teams identify who would benefit from a Care, Education and Treatment Review
