Dynamic Support Register (DSR)

The Dynamic Support Register (is for people who have been diagnosed with a Learning Disability, or Autism, or both)

It is important that local NHS services understand the needs of people with a Learning Disability, Autism or both in their area, so that their care network can be proactive in making sure they receive the right support at the right time.

The DSR for children in Greenwich holds information about the young person and what areas of their care may require enhanced community support. The information helps services to work more closely together to manage a person's support effectively and ensure good outcomes in the local and wider community.

What is the benefit of being on the Dynamic Support Register?

Being on the DSR means that it may help professionals to understand your/your child’s needs and help in recognising early signs that might lead to a crisis. Being on the DSR means that extra support can be put in place quickly such as the SELECT Key working Support Service. The information helps services to work more closely together to manage a your/your child’s support effectively and ensure good outcomes in the community.

Who should be on the Dynamic Support Register?

An individual should be added to the register if they meet one or more of the following criteria. Signs of being at risk of admission will be different for every individual.

·         Identified for pre-admission or community Care, Education Treatment Reviews              (C(E)TR)

·         Identified for or under the Care Programme Approach (CPA) process

·         Under the care of behavioural/early intervention service, e.g. youth offending

·         Recently discharged from hospital

·         Young people in residential school placement

·         Presented 'in crisis ' at A&E departments Under the Home Treatment Team (HTT)

·         Presenting with significant behavioural challenges

·         Being supported in an unstable environment or concerns about longevity of the              placement

·         Quality concerns or safeguarding alert has been raised

·         Significant life event and/or change

·         Has no family carers/advocates

·         No effectively planned transition from children to adult services

·         Child or young person who has been excluded from school subject to regular                  and/or prolonged restrictive practices

·         Has been in contact with the Criminal Justice System

·         Has drug and alcohol addiction problems

·         Has unstable/untreated mental illness

·         Previous history of admission(s)

·         Not previously known to services

·         Non-compliance with medication

·         Not attending/being brought to appointments

·         Child or Young Person in Care

What information will be held about you/your child on the Dynamic Support Register?

The information will depend on you/your child’s  care and support needed. Usually, it will include information such as:

  • Name of you/your child
  • NHS number (if known)
  • Place of education
  • Information about who is involved in your/your child’s care
  • How they are managing with their Health, Education, and social circumstances
  • Any Specialist Care Plans that have been developed to support them living in the community

What do we do with the information on the Dynamic Support Register?

The Dynamic Support Register for Children and Adolescents in Greenwich is managed at present by Greenwich’s Children and Young People’s Commissioning Manager. Referrals for the Dynamic Support Register can be made to the Commissioning Manager who will then place the child on the DSR.

Once your child has been identified and added to the DSR, their care will be routinely reviewed in combination with their support network to make sure that all of the best practice for young people with a Learning Disability and/or Autism is in place.

We know that the needs of young people can change quickly, so this register is intended to aid early identification of increased risk and help us to proactively support them in the community.

Eligibility Criteria

• Children and young people up to the age of 18
• Diagnosis of Autism and / or a Learning disability
• Presenting with significant behaviours of concern

Sub-category

• Risk of placement breakdown i.e. home environment, educational setting, placement settings
• Risk of hospital admission (psychiatric) or
• At risk of requiring more intensive support to access the community

What if you/your child changes their mind about being placed on the Dynamic Support Register?

Parents or the young person can withdraw consent for their child’s details to be held on the register at any time and we will approach them every year to check if they still agree to consent.
